I have found Diskeeper to be the most efficient. Its also very easy to use as it can be set to defrag automatically in the background, has a nice GUI and defrags system files.
Diskeeper 2009 - it's easily the best defragger for Windows. Fast, powerful, and fully automatic. It can defrag system files, multiple disks simultaneously, defrag in low space ( < 5%) etc etc and is fully automatic too.

The built in disc defragmenter in Windows is just fine.
