I think in some episodes maybe they can, I don't remember exactly. But mostly I'm pretty Brian and other young-ins can understand him and some people outside of his family. Well, actually I do remember one episode where he goes to High School and people seem to understand him.
Whether or not people can understand Stewie is a running gag in the show. Obviously Brian can understand him, Bertram, Stewie's brother can....and for most of the show's run the family doesn't really respond to Stewie. Seth MacFarlane once said that he is like most other babies in the world. He probably has all these thoughts but when he tries to put them into words he just sounds like a silly little baby. So Brian and the audience can hear what are basically Stewie's thoughts, but the other characters hear those thoughts filtered through a small toddler's mouth. Although there are a few choice instances where the family can hear Stewie. Prompting Stewie to say, "Oh, so NOW everyone can understand me?!". And in another episode, Brian and Stewie are about to talk to Cleveland's ex-wife and they are discussing whether or not she is one of the characters that can understand Stewie. Brian sums it up to "She'll understand the basic gist of what you're saying". The continuity is a little off when it comes to Stewie's verbal interactions but I think I have explained it pretty well.

In multiple episodes he talks to peter, chris and meg as well as brian. in one episode he is an actor who performs with a girl named olivia, if people cant understand him why would they pay good money to see a baby speak gibberish. It seems like the only person he has not had a conversation with, is lois. usually he is ignored though.

Stewie's dialog is for the benefit of the television audience. He also talks to his teddy bear and other children but adults have a hard time understanding him or often only get the gist of what he saying.
They've given the two characters in the family who normally cannot talk (a baby and a dog) the ability to communicate with each other and they let us in on it. That's part of what makes Family Guy one of the best comedies on television.

Yes! Especially in the "road trip" episodes, the time travel stories, or any instance where he is actually away from the rest of the family. My feeling is that his family see him as a normal baby and just don t hear him.
