Fill out the places to be filled out. Then take the packet to city hall, or town hall, or even the post office, (take your parents with you) and show your ID to make sure you are who you say you are and they stamp it. It makes the document legit.
You need to go to a notary public and swear that the information on the cards is correct. You can usually find notaries at banks and stores that provide shipping and copying services. You might want to copy the cards so each one is on a sheet by itself. Whoever designed the form did not leave room for the notary's seal. The last time I had something notarized in Texas, back in 2000, the notary used a seal.
Be sure to bring photo ID, such as a driver's license, to prove your name to the notary. Make sure the name on the card matches the name on the ID.
