If someone had a sex change, would they still have to sign up at the post office at 18 AND if they had the sex change after they signed up, would they still have to go if a draft was in effect?
Actually a sex change disqualifies someone from serving even if they want to. The question of prior or post enlistment is irrelevant. They are considered to be not fit for service. The same applies to anybody missing any organs (ie. a kidney) as they are 'using up' the slot for a healthier individual. I personally don't understand the logic behind it but those are the regulations.
The military is currently exceeding their recruitment goals and is being especially selective with the air force. There is no shortage of willing people especially given the state of the economy. That, when taken with the fact that drafted people make horrible soldiers, indicates that no draft will be put in place so the point is moot.
Most likely, a person 18 years old would be denied a sex change operation, but if they were drafted after the op. they would have to report, and most likely given some no combat post
They would have to register based on their birth gender. They would be medically disqualified from serving after reporting for a physical. The medical standard is the source below.
They should want to defend this country, not feel they "have" to. Thats what is wrong with this generation. No one wants to do anything unless something is in it for them. Lazy...

You were born male . . . all males must sign up for the draft . . . therefore you must sign up for the draft.
Once the "changed man" shows what's under the skirt . . . the issue will me done.
There is no draft right now . . . so . . . it's pretty much an academic point.

There will never be another draft in this country during your lifetime. Don't worry about it.
