I used to see on TV, 25 years ago, women's uneven parallel bars were in a different configuration than they are today (closer together). When did they change, and did everyone change at once, or did a few mavericks change them for their routines?

Well, you're kind of right. Coaches began to tinker with the uneven bars so their gymnasts could do more difficult swings and release moves. Gymnasts, such as Korbut and Commaneci, began to make moves...and then more followed in suit and the giant swing became. It was inducted into the COP and soon it became a norm so it was necessary for the bars to be set far apart so the gymnast could swing at ease.
Uneven Parallel Bars
well one of the main reasons for the change in the uneven bars was so gymnasts could preform the giant swing which is a full circle around the bars. it is a very reacent change but now that it is changed gymnasts are able to preform harder skills and they keep getting harder.
I remember they used to come over the top and slam their pelvis into the lower bar. I m glad the greater distance between the two bars now precludes that.
