I presume you are sending flowers with the enclosed message. Just make a short note like: "Our deepest condolence in your hour of bereavement".
I think 2 and 3 are the best. 2 if you dont know the family very well, 3 if you know them very well. They sound more genuine and it really is true that you can never say anything to comfort someone who's lost a loved one.
if you know everyone in the family and it's about 4 or 5 members it's always more personal to mention each of them. If not I guess its ok to say family smith family duff
