Yes, you normally give a gift at a Bar Mitzvah. It can be any normal type of birthday present, or it can be a check or gift card. You don't give it to the boy at the synagogue, but wait until after the service is over and you go to the reception or party. There are Bar Mitzvah cards at places that sell greeting cards, or you can give any card that say "Congratulations" or "Happy Birthday". She can also write "Mazel Tov!" inside the card, which is the Jewish expression for congratulations.
Your sister should wear a dress that covers her shoulders and comes down to about the knees. Since it's casual, the dress shouldn't be overly fancy.
I'm not sure about what gift you bring. You never can go wrong with a card filled with some money (which is what I think most bring).
Far as what to wear, she should wear a nice modest DRESS. Nothing too tight or revealing, its disrespectful.
