It could be lots of things but I don't hear much of diabetics constantly vomiting. I vomit after eating bad meat or fish, or mixing milk with lemon or something. So I think your brother has a different origin of cause than diabetes, consult with his care team.

They could go into diabetic ketoacidosis (DKA).
Check him for ketones and seek medical attention immediately!

It can be gastroparesis; which is gastric atony with delayed emptying and unpredictable absorption.
