Your cough probably isn't the cause to your pain in your shoulder. You shoulder might be injured and when you cough, the vibrations hurt your shoulder. You should still go see a doctor, and if you cant go chase down a med student and ask him or her
You just need to go to the Doctor and have them check you out.
